The platform
Fortem Group LLC, doing business as Fortem Health provides a telehealth platform that connects members with independently licensed physicians and nurse practitioners. When a provider prescribes medication, the prescription is sent to an independent licensed pharmacy of the member’s choosing or one we help coordinate. Fortem Health does not practice medicine, employ providers, or operate a pharmacy. All clinical decisions are made solely by licensed providers.
Eligibility
You must be 18 or older and located in a state where Fortem operates. Availability depends on provider licensing in your state.
Membership and fees
Membership is billed monthly and covers your clinical care: provider oversight and visits, lab review, care team messaging, care coordination, and plan adjustments. Membership does not include medication. There is also a one-time setup fee of $149. The current membership price is $119 per month. One membership covers every program a member is enrolled in (testosterone, sexual health, hair, thyroid, women’s hormone, and weight care) at no additional membership cost.
Medication and pharmacies
Fortem Health does not sell, dispense, or bill for medication. If your provider prescribes medication, the prescription is sent to an independent licensed pharmacy. That pharmacy fills the prescription, bills you directly at its own price, and ships to you. Any cost estimates we show are approximate; the pharmacy sets the final price. Fortem receives no markup or fee on medication.
Setup fee, cancellation, and refunds
The one-time $149 setup fee is paid when a member reserves their provider visit, before labs and the visit. If the provider determines treatment is not appropriate, $100 of the setup fee is refunded. The setup fee is non-refundable if the member misses the visit or cancels with less than 24 hours’ notice; one reschedule with at least 24 hours’ notice is allowed. If required labs are not completed within 60 days of reserving, the reservation expires and $100 of the setup fee is refunded. The monthly membership begins only after provider approval. Laboratory services are billed separately by the lab at its published rates; Fortem does not mark up labs. You may cancel your membership at any time; cancellation takes effect at the end of the current billing period, and membership fees already billed for the current period are not prorated. Medication charges are between you and the pharmacy and are governed by the pharmacy’s policies.

The platform is not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911.
Your responsibilities
You agree to provide accurate health information, to follow your provider’s instructions, and to keep your login credentials secure.
No guarantee of outcomes
Individual results vary. Nothing on the platform is a promise of a particular clinical outcome.
Intellectual property
The platform, its design, and its content are owned by Fortem Group LLC, doing business as Fortem Health or its licensors.
