If your provider prescribes something, a licensed pharmacy fills it, ships it and bills you directly. Fortem adds no markup.
Need new labs? You pay the lab directly. Already have recent results? Share them at no cost.

Pricing questions
Is medication included in the membership?
No. Your membership covers your care. If your provider prescribes medication, a licensed pharmacy fills it, ships it and bills you directly at its own price. Fortem adds no markup and never charges you for medication. You’ll see an estimate in your plan summary before you pay anything.
What does the $149 setup fee cover?
Your provider’s review of your intake and labs, your video visit, and lab coordination. It’s paid once, when you reserve your visit.
What if my provider decides treatment isn’t right for me?
You get $100 of your setup fee back, and your membership never starts. You also get one free reschedule with at least 24 hours’ notice. If labs aren’t completed within 60 days, your reservation expires and $100 is refunded.
When does the monthly membership start?
Only after your provider approves your plan. Nothing is billed monthly before then.
Can I add another program later?
Yes. One membership covers every program you’re enrolled in, so adding testosterone, hair, thyroid, women’s hormones or weight care later costs nothing extra.
What about labs?
If you have labs from the last three months, share them at no cost. If you need new ones, we order them and you pay the lab directly, at its price.
Can I cancel?
Anytime, from your portal or by messaging your care team. Cancellation takes effect at the end of your current billing period.
